Greetings to you—both new and returning Bethany Lutheran College students. This Web page is designed to provide up-to-date information and answer many of the questions students have before the beginning of the new semester.

Form instructions

Health History, Physical Examination, & Clearance Forms

Print off a copy of these three (3) forms and then take them with you to your doctor. We will not accept a physical or health history done on any other form. Your physical, health history and clearance form need to be completed during the summer before you start competing at BLC, so from May-August. These forms must be signed and dated by the physician who performed the physical.

Emergency / Insurance Form

Print off a copy of this form, then please read the form carefully, and fill it out completely making sure all blanks have been completed.

Please note that if your insurance policy is an HMO/PPO, please check with your insurance company about changing your clinic to a participating clinic in Mankato. Some insurance companies allow for this in order to receive “in network” benefits. If your HMO/PPO does not have this option, please locate “out of network” medical facilities in the Mankato area and attach a list to this form. This information is IMPORTANT in order for the BLC Medical Staff to provide the best care possible for you.

All students are required to show proof of insurance prior to the beginning of the academic school year. Insurance coverage is the responsibility of the student athlete as Bethany Lutheran College does not carry a secondary policy covering athletic related injuries. Please make two copies of your health insurance card (front and back) and send them along with the forms.

Print off a copy of this form, then please read the form carefully, and fill out the appropriate section. Fill out the top section (i.e., Parent Version) of the form if you are covered under parental insurance. Fill out the bottom section (i.e., Student Version) of the form if you have independent insurance, not under parental insurance.

Waiver / Consent Form

Print off a copy of this form and then please read the form carefully. At the top of the page in the appropriate spaces, please list the sport(s) in which you may participate and print your name. Please initial each section (i.e., 4 spots) and sign at the bottom of the form that you have read and agree to the information. If you are under 18 years of age, your parents must also initial and sign. If you choose to refuse to sign any of these, write “Refuse to Sign” beside the appropriate document title.

Student Information Sheet

Print off a copy of this form and fill it out completely. It is very important that all areas of the form are completed. The information on this form is used by the Sports Information Department for rosters, media guides, and media contact information. We are also able to send information to your parents and hometown newspapers when our area media covers your athletic events. The Fine Arts Division at Bethany has many opportunities for your involvement throughout the academic year — and we warmly invite ALL of you to explore the options. Whether the arts were a big part of your high school experience or past college activity, or if you just want to explore some new territory, we have possibilities for you! See instrumental music, choral music, visual art, and theatre to learn what's available and what to watch for as you arrive on campus.

Fall 2008 instrumental music opportunities:
The various instrumental groups include: Concert Band, Chapel Brass, Jazz Band, String Ensemble, Drumline, Pep Band, private instrumental lessons.

Concert Band is open to all musicians for campus, chapel, and tour performances, rehearsals 3:30 - 4:20 p.m., Monday - Wednesday. Watch for announcements! The Concert Band and the Jazz Band will tour regionally in March, 2009 during Spring Break. Watch for information regarding the first rehearsal when you arrive on campus in August. Notices will be posted in the Ylvisaker Fine Arts Center as well as information in your email and mailboxes. If you have questions about instrumental music you may contact Prof. Adrian Lo.

Chapel Brass is open to brass members of the Concert Band and will perform for chapel services as well as other campus performances and tour, rehearses Thursdays from 3:30 - 4:20 p.m. Watch for announcements!

Jazz Band is open to all members of Concert Band for campus and tour performances, rehearsals will be determined by the availability of the members and director. The Jazz Band tours with the Concert Band. Watch for announcements!

String Ensemble: Plans and rehearsal information TBA for fall semester. The ensemble has performance opportunities both on and off campus during the academic year. Watch for announcements!

Drumline is a student-directed drum corps that participates during sporting and various campus events, rehearsal times TBA. Watch for announcements!

More information on Pep Band will be available soon.

Students may register for half-hour private instrument lessons (MUSC190D) with the registrar. The cost per semester is $350 for 13 lessons. An instructor and lesson time will be assigned to you based on your class schedule and that of the instructor. Watch the "lesson information" bulletin board in the practice room hallway of the Ylvisaker Fine Arts Center for the lesson schedules to be posted there!

Fall 2008 vocal music opportunities:
Everyone is welcome to find out about the various choirs and groups including: Concert Choir , Mary Martha Singers, Choraliers, Handbell Choir, and Private vocal lessons. Keep your eyes open on campus for more announcements and updates about choral music at Bethany!

Concert Choir is a mixed choir for campus and tour performances. Rehearsals are 4:30 - 6 p.m. Tuesday, Wednesday, and Thursday. Performance opportunities include a choir tour right after graduation in May. Everyone interested is invited to attend a "gathering rehearsal" on Tuesday, August 26 at 4:30 in Silber Hall, room 101 of the Ylvisaker Fine Arts Center. This rehearsal will acquaint you with some of the music that both the Concert Choir and Mary Martha Singers perform and will also provide information about the audition process for both. Auditions will be held beginning at 3:30 on Wednesday, August 27. Watch for announcements!

Choraliers is a non-auditioned vocal organization that performs a lyric opera (usually Gilbert and Sullivan) presentation early in the spring semester. Rehearsals are alternate Mondays from 4:30 to 5:30 for chorus parts. Leads are chosen mid-semester and performances will be January 30 and 31 and February 1, 6, and 7. Watch for announcements early in fall semester regarding beginning rehearsals.

Handbell Choir rehearses are on alternate Mondays from 4:30 - 5:30 p.m. The Handbell Choir performs and tours with the Concert Choir and Mary Martha Singers. Watch for announcements!

Fall 2008 Visual Art opportunities at Bethany:
Bethany students have the opportunity to enroll in a variety of studio art classes including drawing, ceramics, sculpture, painting, digital art, and photography. Consult the course offerings for fall 2008 to find current classes and times.

Check out these fall theatre opportunities:

Theatre Information Night: Anyone interested in any aspect of theatre is invited to attend the Theatre Information Night on Tuesday, August 26 at 7 p.m. in the theater of the Ylvisaker Fine Arts Center. Come and find out about the fall productions specifically as well as Bethany Theatre in general. Everyone is welcome!

Theatre Physics, the annual kick-off for the Bethany theatre department, is a vaudeville-type original production featuring skits and performances created collaboratively by Bethany students and faculty. Auditions for Theatre Physics will be Wednesday, August 27 beginning at 7 p.m. in the Ylvisaker Fine Arts Center. Performances are held Friday, September 26 and Saturday, September 27 at 7p.m. and 9 p.m., Sunday, September 28 at 2 p.m. Watch for announcements!

Shakespeare's classic "Romeo and Juliet" is this year's Fall Play, directed by Mattehew Caron. Auditions for the play will be Wednesday, August 27 beginning at 7 p.m. in the Ylvisaker Fine Arts Center. It will be performed Fridays and Saturdays, November 14, 15, 21, and 22 at 7:30 p.m. and Sunday, November 16 at 2 p.m.

Additional opportunities for involvement include set production, props, lighting, sound, and publicity.

Watch your campus mailboxes, Daily Bulletin announcements, campus bulletin boards (particularly those in the Ylvisaker Fine Arts Center) for up-to-the-minute information regarding auditions and registrations for music and theatre activities when you arrive on campus.

On behalf of the entire residential life staff, I welcome you to Bethany Lutheran College.

Living on campus will be a significant part of your college experience. You will grow in new ways, meet lifetime friends, and create many memorable moments. The residence halls are staffed with Resident Mangers (RMs) and Resident Assistants (RAs). The RMs live in an apartment in your residence hall and are there to help, guide, comfort, and listen. RAs are upper division Bethany students who live on each wing, staff the front desk, and assist students as needed.

When you arrive on campus, your RA will direct you to your room, issue your key and fob, and check you into the residence hall. You can pick up your welcome folder from your RM. For guidance on outfitting your room, read the list about what to bring to campus. Click here for your housing assignment.

Once you settle into your new home, I encourage you to become as involved as possible at Bethany. Opportunities abound for meeting new people, learning new things, and exploring your creative and physical self through intramurals, student activities and events, student government, the fine arts department, athletic events, etc.

Welcome, from Information Technology Services. We are available to answer calls from students and parents, especially for advice on what computer to buy, what software is needed, or how to reset a password. The common questions page contains a lot of useful information.

What computer to buy:
We support both Windows and Apple Macintosh computers, so purchase whatever you are used to using. Most of the Windows computers owned by Bethany are Dell brand, but there are other good brands out there as well. Competition between vendors keeps prices fairly low. Apple, for example, offers education pricing: visit apple.com/education. Many students bring laptops, which usually contain a wireless networking card. If yours does not, we recommend that you install one.

What software is needed:
Many vendors are able to provide education pricing for software. If possible, have Microsoft Office installed. There are free alternatives such as OpenOffice.org.

When you arrive on campus:
To keep your computer and the Bethany network safe, we will install software on your computer before you access our network. This Safe Access software will ensure that your computer is virus-free and up-to-date with security patches. We will also give each student a free copy of Norton Antivirus. You are welcome to use other leading brands of antivirus software as long as the licensing is current.

Your login and password information will be mailed to you. Please check your blc.edu email account periodically as we will send important notices there and so will faculty and BLC administrators.

Dean of Students Ted Manthe requests that parents of new students attend either of the two brief parent orientation sessions. Both sessions are the same, so come to the one that fits your schedule.
