Under the provisions of the Family Educational Rights and Privacy Act of 1974 students have the right to withhold the disclosure of any or all directory information listed:

 2.  Local Address/Phone
 3.  Permanet Address/Phone
 4.  Email Address
 5.  Date and Place fo Birth
 6.  Hometown
 7.  Degrees and Awards Received and Dates
 8.  Dates of Attendance (Current and Past)
 9.  Full or Part-Time Enrollment Status
10. Participation in Officially Recognized Activities
11. Participation in Officially Recognized Sports
12. Weight/Height of Members of Athletic Teams
13. Most Recently Attended Education Institute
14. Major Field of Study
15. Academic Level
16. Residency Status
17. Photograph

At Bethany we normally compile a student body list, send information to your hometown newspaper and validate your enrollment for financial aid purposes. If you do NOT want your name printed, or this information released, please print and sign the REQUEST TO PREVENT DISCLOSURE OF DIRECTORY INFORMATION form and return it to the Registrar's Office.

Remember only print, sign and return this form if you do not want information released.
